Owner Permission for

Animal-to-Animal Contact

Section 1: General Animal to Animal Contact Permission

I, the undersigned owner or authorized agent of the above-listed animal(s), hereby give Zen Petcare & Home permission to allow my animal(s) to have supervised contact and interaction with animal(s) owned by other clients of the facility.

 I understand and agree to the following:

• This permission is voluntary and can be revoked by me in writing at any time.

• This permission will be valid for one year from the date signed and must be renewed annually for continued participation.

• While Zen Petcare & Home takes reasonable precautions to ensure the safety of all animals, I understand there is always some risk associated with animal-to-animal interaction, including possible injury or disease transmission.

• If I request that my pet(s) be kept completely isolated from all other animals, I understand that this boarding facility may not be the appropriate fit for my needs.

• I release Zen Petcare & Home, its owners, employees, and agents from liability for any injury, illness, or other harm arising from such interactions, except in cases of gross negligence or willful misconduct.

Section 2: Puppy Comingling Permission

(Dogs Under 6mos)

Complete this section ONLY if your dog is under 6 months of age and you wish to allow them to have contact with adult dogs during their stay. Per NC 02 NCAC 52J .0209(4), written owner permission is required for puppies under 6 months to have contact with adult dogs other than their own dam.

I understand that my puppy is under 6 months of age and I specifically request and give written permission for Zen Petcare & Home to allow my puppy to have supervised contact and interaction with adult dogs during their stay.

I further understand and agree:

• Puppies are still developing physically and socially, and interactions with adult dogs carry additional risks including injury, stress, and disease exposure (particularly for puppies not fully vaccinated).

• My puppy's vaccination status is current as documented on file, and I understand certain communicable diseases pose a higher risk to young dogs.

• Zen Petcare & Home will use their judgment in selecting compatible adult dogs for any comingling and may decline comingling at any time if safety concerns arise.

• This permission is specific to comingling with adult dogs and is separate from general animal-to-animal contact permission in Section 1.

• This permission must be renewed annually or when my puppy reaches 6 months of age, whichever comes first.
