This is to inform you that I have contracted the services of Zen Petcare & Home to provide pet-care services for my pet(s). This is a standing authorization on file.
Should my pet(s) require veterinary attention while in their care, I authorize my veterinarian (listed below) to extend treatment and to share my pet's health and treatment information with Zen Petcare & Home. If my veterinarian is unavailable, I authorize Zen Petcare & Home to seek treatment at the nearest appropriate veterinary emergency clinic.
I accept financial responsibility for all emergency care of my pet(s).
